Medical Consent Forms

To all camp attendees/parents,

Everyone who is attending camp whether child or adult must fill out a medical form

Here are a few things that we want to point out and clarify:

  • In the 1st paragraph of the form, it gives us permission to seek medical attention in the case of an emergency including blood transfusions. If you do not give the emergency personnel permission for blood transfusions, please put a line through it and initial above the line.
  • If a parent is traveling with child and will be administering medications to their own child then please state that on the child's form.
  • If you are over 18 and take medications, they need to be kept on your person or safely secured at all times. This is for your safety as well as others.
  • In the event of an emergency where medical decisions need to be made – please list an emergency contact at the bottom of the form.
  • We will have Tylenol/Ibuprofen/Pepto/Antacids and other OTC (over the counter) medications in the medical kit however please put on form which medications can be administered and dosage. Reason for this is due to unknown past medical history and side effects with other medications. We must have permission to administer OTC medications.
  • Please advise if any medical and/or mental health  concerns with yourself or your child that we need to be aware of. Everything that is discussed is confidential and will only be shared with emergency personnel if needed.
